Notice of nondiscrimination

Free communication assistance

When required, we provide appropriate auxiliary aids and services free of charge to people with disabilities to communicate effectively with us. These may include qualified sign-language interpreters and information in accessible formats.

When required, we also provide language assistance services free of charge to people whose primary language is not English, including qualified interpreters and translated information.

File a grievance

If you believe we failed to provide these services or discriminated in another way, you may file a grievance in person or by mail, fax, or email using the coordinator contact information above. Assistance filing a grievance is available upon request.

File a federal civil-rights complaint

You may also file a civil-rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through its complaint portal or by mail or telephone using current contact information published by HHS.

Notice of availability

Language assistance services and appropriate auxiliary aids and services are available free of charge when needed.

Translated notices

The notice of availability must be displayed in English and, for covered entities, in at least the 15 languages most commonly spoken by individuals with limited English proficiency in the relevant state or states.
